The “Hesemann Method” for Starting a Rhexis

Grab ‘n Go” Rhexis

The “grab ‘n go” method to start a rhexis flap saves a trip in and out of the eye and a few moments. I’m not the fastest now but I’m excited to be learning these efficiency tips early.

“Foldover” Rhexis Technique

Fold the flap of a rhexis over itself before letting it go to keep the material in the central “safe zone” and to give yourself an edge to re-grasp.

Limbal Relaxing Incision Paired with Phaco Wound

YouTube Link: https://youtu.be/9m4tgAdI5QM

Limbal Relaxing Incisions (LRI) are cuts in the peripheral cornea made on the steep axis of astigmatism to “relax” the cornea and reduce astigmatism. These can be paired with phaco wounds which are usually placed temporally 180° in patients with against the rule (ATR) astigmatism. For with the rule astigmatism incisions are usually made superiorly and inferiorly on the cornea.

Using the I/A As the Second Instrument

Entering “dry” with the I/A handpiece (no irrigation) after implanting the IOL and using is as an instrument to position the IOL before engaging irrigation and removing OVD also saves a trip in and out of the eye.
